What’s Included?

Preventive:

Adult Cleaning – 100% (2 per year)
Child Cleaning – 100% (2 per year)
Fluoride Treatment – 100% (1 per year)
Additional Cleanings Per Year – 15%

Restorative Procedures:

You will receive 15% off all treatments including sealants, fillings, crowns, implant placement, implant crowns, Invisalign, dentures, partials, etc.

Diagnosis and Radiography (X-rays)
Comprehensive Exams – 100%
Periodic Exam – 100%
Full-Mouth Series of Radiographs or Panoramic – 100% (1 every 3-5 years)
Bitewing X-rays – 100%
Limited, Problem-Focused Exam – 100% (Limit 1 per year)
Limit is set at 2 basic exams and 1 limited exam per year. Any additional exams are not included.
